An Explanation of Benefits (EOB) is a statement from your health insurance company—not a bill—detailing how a medical claim was processed. It shows the services you received, the provider’s charges, the amount covered by insurance, and your final, potential out-of-pocket responsibility. EOBs help track deductible, copay, and coinsurance payments.

Key Components of an EOB:
  • Patient & Provider Info: Who received care (name, policy number) and which doctor/facility provided it.
  • Service Details: The date, description, and codes for services rendered.
  • Total Charges: The amount the provider originally billed.
  • Allowed Amount: The negotiated rate between the insurer and in-network provider.
  • Insurance Payment: How much the plan paid.
  • Your Responsibility: What you may owe after insurance, including deductibles, coinsurance, or non-covered services.
  • Transparency: To show you what was billed and how the plan applied benefits.
  • Verification: Allows you to review charges for accuracy and detect potential fraud.
  • Deductible Tracking: Shows how much you have contributed towards your annual deductible.
  • It is not a bill. You will receive a separate bill from your doctor or hospital for the actual amount owed.

Tipically, you should recive an Explanation of Benefits (EOB) from your insurance company 30-45 days after your visit. This will explain what they covered based on the services rendered at the time of the visit.

You may then recive a bill from All Day Medical Care Clinic. The amount is based on your insurance plan / deductible and is determined by your insurance company. Your bill is based on the level of care provided. Bills are submitted to your insurance company using the All Day Medical Care Clinic billing team.
