To request access to your medical records, please contact our office to obtain access to your patient portal or to submit a formal medical records request.

Policies
Please review the following policies before submitting your request to ensure proper handling of your medical records.
Records requested for continuity of care will be sent directly to the designated provider, facility, or authorized recipient.
Patients will not be provided copies of records to personally distribute to third parties.
Psychotherapy notes are protected under Maryland and federal confidentiality laws and may be withheld or redacted to protect patient safety and privacy.
Patients may request documentation including intake forms, dates of service, medication lists, and discharge summaries.

Patients are encouraged to utilize the patient portal for convenient, secure electronic access to their health records.
View your records online at any time. For printed or formally processed copies, records may require clinical or legal review prior to release. Fees may apply in accordance with Maryland state law and federal guidelines.

Processing
To ensure efficient processing, please have the following information ready when submitting your request.
Provide the date range for the records you need, including specific visits if known.
Include the name of the treating provider or department associated with your records.
For third-party releases, provide the full name, address, and contact information of the recipient.
