PCP General Info

Your Primary Care Provider (PCP) will coordinate all routine care for you and your family, and arrange for specialty care, if needed. Your primary care provider will also arrange for hospital care if necessary.

Primary Care at All Day Family Care includes several categories:

  • Family Medicine providers care for individuals at all stages of life, from birth through adulthood.  
  • Internal Medicine providers specialize in the care and treatment of adults.
  • Pediatrics providers specialize in the care of infants, children, and adolescents.
  • Women’s Health providers provide routine screening and testing in addition to ob/gyn care when necessary.
  • Geriatric Care includes comprehensive care programs for seniors.

Unless it is an emergency, you should always call your PCP first when you or your family member gets sick.

Patient-Centered Medical Home Initiative

All Day Family Care is committed to creating a Patient Centered Medical Home for you. A medical home is not a building, house, or hospital, but rather an approach to providing comprehensive health care. A medical home is called a “home” because we’d like to be the first place you think of for all your medical needs. Our goal is to make it easy and comfortable to get the care you need in a way that works best for you.

Your medical home team consists of your primary care provider, a dedicated team of health care professionals, and most importantly, you and your family. As a crucial member of the team, you will be involved in decisions about your health and health care. Your primary care provider will work with you to coordinate your care across the health care system.

Integrated Care Services

All Day Family Care has placed special emphasis on the coordination of primary care and behavioral health services for those who need them. Our vision is to promote and support optimally effective and productive mental and physical functioning in all of our patients.

To this end, each one of our primary care teams has behavioral health providers who practice on the team alongside their medical counterparts. Your providers work together to ensure that you get the best care possible.

Family Medicine

Our family medicine providers specialize in caring for the needs of your entire family throughout all stages of life.  Family medicine differs from internal and pediatric medicine because your entire family can consult with one provider.  When you are healthy, we will help you stay healthy through regular preventive care and health education.  We are also experts in the diagnosis and treatment of illness and the management of chronic diseases. 


Providers at All Day Family Care will take care of your children from birth, through their school age years, and into young adulthood. Your pediatric provider will assess your child’s health care needs, provide health education, and offer resources and support as your child grows and develops throughout the years. Our providers are State certified in early periodic screening, diagnostic and treatment (EPSDT) for pediatric patients (https://www.medicaid.gov/medicaid/benefits/epsdt/index.html)

Our Pediatric Services Include:

  • Physicals, well child care
  • Sick visits
  • Behavioral health
  • Health education
  • On-site laboratory services

All Day Family Care also has pediatric specialists in Behavioral Health

Internal Medicine

All Day Medical Care providers specialize in the treatment and care of adults. When you are healthy, we will help you stay healthy through regular preventive care and health education. We are also experts in the diagnosis and treatment of illness and the management of chronic diseases.

Geriatric Medicine

All Day Family Care family physicians will care for you throughout your adult life and will continue to meet your needs as you enter your senior years.
